Welcome to the new Lakeland webpage for student/parent registration! This is the first year parents will be able to complete most district registration documents online, in the comfort of their own home! We hope you find this initial effort, on the part of the district to utilize technology to complete some of the more time consuming student registration requirements, most beneficial! As the user moves through this webpage, he/she has the option of printing all registration documents and completing them by hand, or actually typing in all information and then printing the completed documents. Parents still need to return the completed documents to their students’ schools to finish the enrollment process.

Requirements for Students new to the District
Idaho Law requires proof of identity and birth date of all new students. The proof is required within thirty days of registration and must be by certified copy of the birth certificate or other reliable proof of a student’s identity, such as a passport, visa, or other governmental documentation of the child’s identity. However, such other forms of identification must also be accompanied with an affidavit explaining the inability to produce a copy of the birth certificate.

Proof of residency is also required for students enrolling new to the district. Acceptable proof of residency may include any document that definitely establishes residency within the school district (i.e. utility bill, property tax receipt, etc.)

To simplify the registration process, parents should plan to provide the required proof of birth date, identity, and residency at the time of registration.

Fall 2007 Immunization Requirements
Students will not be able to attend school without one of the following: 1) proof of completed immunizations; 2) verification of an immunization plan in progress; or 3) a “certificate of exemption”, which must be signed by a parent or guardian. (“Certificate of exemption” forms are available in each of the school offices.)

Immunization documentation must include the date and type of each dose administered and must be stamped by a physician or the health department. A copy will be placed in the student’s permanent file. Immunization requirements for kindergarten through second grade students include; a minimum of (5) doses of DTP, (3) doses of Polio, (2) doses of MMR (measles, mumps, and rubella), and (3) doses of Hepatitis B. Immunization requirements for third through twelfth grade students include; 4-DTP, 3-Oral Polio, and 1-MMR. Three doses of Hepatitis B vaccine are also required for all children born after November 22, 1991.

Attendance Zone Transfers
Parents of students who are residents in one Lakeland attendance zone and wish to have their child attend another Lakeland zone/school must apply annually. Students with attendance zone transfer applications on file from last year and who have completed zone transfer application for the coming year may register in the school they last attended. Students applying for an attendance zone transfer for the first time will be required to register in the school of the attendance zone wherein they reside. Enrollment trends across the district observed during registration will be considered in granting final approval for any zone transfer applications.
